Hormone-informed psych evaluation near Hollywood Beach Broadwalk and downtown Hollywood
We document sleep architecture, menstrual or androgen history, thyroid clues, and substance use because they change the differential.
What Psychiatric Evaluations actually includes
Adult ADHD assessment uses developmental history, ASRS, and mimics (anxiety, trauma, sleep, thyroid). A childhood report card helps; a viral video does not.
Anxiety evaluation separates GAD, panic, social anxiety, OCD, PTSD, and medical mimics such as hyperthyroidism or stimulant toxicity.
Indications and differentials
Insomnia that is either a syndrome of its own or a driver of daytime psychiatric symptoms.
New or unexplained mood, anxiety, attentional, or psychotic symptoms lasting beyond a brief adjustment reaction.
Family history of bipolar disorder or psychosis that changes how aggressively we screen.
Not indicated solely to obtain a letter that the interview does not support.
From intake to follow-up: Psychiatric Evaluations
Mental status exam is documented: appearance, speech, affect, thought process, insight, cognition, and risk.
If telehealth was used, we record why an in-office visit was or was not required.
Safety: if risk is high, we stop the routine template and use crisis pathways.
Before the visit: intake forms, medication list, prior labs. Incomplete forms shrink the diagnostic hour.
Monitoring and safety in Psychiatric Evaluations
Patients can request a copy of the formulation; mystery diagnoses are not a clinical style.
Collateral from a partner or parent is requested when insight is limited — with consent.
QT, sodium, TSH, and metabolic labs when the chosen medication class requires them.
Higher level of care is recommended when outpatient Psychiatric Evaluations cannot hold the risk.
Why Hollywood presentations look the way they do
Psychosis, OCD, or eating-disorder questions were skipped because the template did not include them.
PHQ-9 was never scored, or it was scored once and never repeated, so nobody can tell whether treatment is working.
Chronic pain, shift work at the port or airport, and hurricane displacement in Broward County never made it into the history.
Clinical FAQ
Can you evaluate psychosis?
Yes, and we will escalate to a higher level of care if outpatient treatment is unsafe.
Do you replace my therapist?
No. Psychiatric evaluation and psychotherapy are complementary.
Do you see children?
The practice is primarily adults and older adolescents, considered case-by-case.
What labs might you order?
Hypothesis-driven: TSH with free T4, ferritin, vitamin D, B12, folate, CBC, CMP, and selected hormones. Not every patient needs every test.
Is the visit confidential?
HIPAA applies, with the usual safety exceptions (imminent harm, mandated reports).
How soon is follow-up?
Often 2–4 weeks after a new medication or a complex formulation; longer when stable.
What we do not do
Psychologists (PhD/PsyD) provide testing and psychotherapy. Dr. Dorothy Chatelier, DNP, FNP-BC, PMHNP-BC provides medical psychiatric evaluation and prescribing within Florida APRN scope.
Traditional 15-minute med checks assume the diagnosis is already correct. We do not assume that.
Therapy intake focuses on process and alliance. Psychiatric evaluation focuses on diagnosis, risk, and medical treatment options.
