Clinical scope of Psychiatric Evaluations in Broward County

Sleep assessment looks at insomnia, circadian delay, apnea clues, and whether poor sleep is driving the daytime syndrome or following it.

A psychiatric evaluation is a diagnostic procedure: history, mental status, risk, collateral when needed, and a differential. It is not a personality quiz.

Evidence we actually use

The Hollywood office is the only brick-and-mortar site. Psychiatric Evaluations for Hollywood is service-area care from that office plus telehealth — not a separate clinic in every city.

Dr. Dorothy Chatelier, DNP, FNP-BC, PMHNP-BC holds DNP, FNP-BC, and PMHNP-BC credentials. Psychiatric Evaluations is advanced-practice psychiatric care, not a 15-minute refill mill and not unsupervised coaching.

The practice does not publish fabricated star ratings. Patient stories on this site are process descriptions, not purchased testimonials.

Clinical FAQ

Can you evaluate psychosis?

Yes, and we will escalate to a higher level of care if outpatient treatment is unsafe.

Do you treat ADHD in adults?

Yes, with a differential that includes sleep, trauma, mood, and thyroid mimics. Childhood history still matters.

How long is a psychiatric evaluation?

New evaluations are typically 60–90 minutes. Follow-up length depends on risk and how many variables changed.

Is GeneSight required?

No. It is considered after failed medication trials or significant side effects.

Will you prescribe on visit one?

Only when the diagnosis, safety, and monitoring plan support it. Controlled substances have a higher bar.

Do you diagnose on the first visit?

Usually we can state a working diagnosis and differential after the first complete interview. Some presentations need collateral, labs, or a second visit.

What changes if Psychiatric Evaluations is done thoroughly

  • Informed consent that lists alternatives, including no medication.

  • An evaluation long enough that you do not have to triage your own symptoms in the parking lot.

  • Perinatal and hormone-linked mood history taken as medical history, not as personality.

  • Space to disagree with a prior label without being treated as noncompliant.

  • Documentation that would survive a covering clinician, an insurance review, or a move out of Hollywood.

Monitoring and safety in Psychiatric Evaluations

We do not combine MAOIs with contraindicated agents. We do not ignore serotonin syndrome risk.

Follow-up interval tightens when a new antidepressant, antipsychotic, or mood stabilizer is started.

QT, sodium, TSH, and metabolic labs when the chosen medication class requires them.

PDMP review before controlled substances. No stimulant start from a chat message.

DSM-5-TR framing for Psychiatric Evaluations

Pediatric primary care is not the focus of this clinic; adolescents are considered case-by-case.

A request for a second opinion before a major medication change or a disability form.

Perimenopausal, postpartum, PMDD, or androgen-related mood change that has not been medically reviewed.

Insomnia that is either a syndrome of its own or a driver of daytime psychiatric symptoms.

What we do not do

ER psychiatry stabilizes crises. Outpatient Psychiatric Evaluations is for people who can wait for a scheduled diagnostic hour.

Psychologists (PhD/PsyD) provide testing and psychotherapy. Dr. Dorothy Chatelier, DNP, FNP-BC, PMHNP-BC provides medical psychiatric evaluation and prescribing within Florida APRN scope.

A primary-care PHQ-9 is a screen. This evaluation is a diagnostic interview with a mental status exam.

Visit flow for Psychiatric Evaluations

Before the visit: intake forms, medication list, prior labs. Incomplete forms shrink the diagnostic hour.

History of present illness is a timeline, not a mood adjective. We date onsets, postpartum windows, and medication trials.

Medical review flags thyroid, anemia, cardiac QT risk, pregnancy, and neurologic symptoms that redirect the workup.

If telehealth was used, we record why an in-office visit was or was not required.
