GeneSight-informed trials after failed antidepressants
We measure what the drug can harm: sodium, TSH, lipids, A1c, weight, AIMS, pregnancy — class by class.
Medication Management for Hollywood patients
Booking a med check is booking a medical visit, not a vending machine.
Tapering is scheduled. Paroxetine, venlafaxine, benzodiazepines, and stimulants each have different withdrawal physics.
What changes if Medication Management is done thoroughly
Coordination with HRT/TRT so endocrine and psychotropic plans are not accidental.
Mood-stabilizer levels when the drug has a level.
Clear instructions for missed doses and alcohol.
Start low, go slow when the class requires it; go to a therapeutic dose when the evidence requires it.
Pregnancy and lactation counseling before the prescription is sent.
Clinical FAQ
What labs do I need?
Depends on the drug: TSH, lithium level, A1c, etc. We will list them in the plan.
Where is the office for in-person med checks?
7603 Davie Road Extension, Hollywood, FL 33024 — about 0 minutes from Hollywood.
Can medication management be telehealth?
Yes if you are in Florida and the case is appropriate. Controlled-substance rules still apply.
Are leftover 'affordable refill' URLs a discount?
No. They are retained slugs. Billing follows insurance contracts.
Can you prescribe if I am traveling?
Only if you are physically in a state where we are licensed — currently Florida telehealth.
What about pregnancy?
Tell us before a prescription is sent. Some agents need specialist coordination.
Protocol elements we do not skip
Missed labs = delayed titration.
Lithium: level, TSH, creatinine, pregnancy test as indicated.
Informed consent stored in the chart.
MAOI diet and washout rules if that class is ever used.
From intake to follow-up: Medication Management
Confirm the target symptom and the diagnosis it belongs to.
Set the next visit before the bottle runs out.
Review PDMP and allergies.
Check labs due for the current class.
Evidence we actually use
Clinical decisions for Medication Management are made by Dr. Dorothy Chatelier, DNP, FNP-BC, PMHNP-BC (DNP, FNP-BC, PMHNP-BC). Pharmacogenetic data (including GeneSight when indicated) can inform medication choice; it never replaces a diagnostic interview.
When Medication Management overlaps with psychotherapy needs, the practice coordinates with therapists rather than replacing weekly therapy. Medication and psychotherapy have different mechanisms and both may be indicated.
Hormone-informed psychiatry means estradiol, progesterone, testosterone, thyroid, and cortisol are considered when the history for Medication Management points there — not as a default panel on every patient.
Indications and differentials
ADHD medication only after the evaluation supports ADHD over mimics.
Bridge care after hospitalization until a stable outpatient cadence exists.
Antidepressants only after bipolar-spectrum screening.
Taper request that requires a medical schedule.
What we do not do
Psychotherapy remains indicated for many diagnoses even when medication helps.
Tapering is slower than patients want and faster than 'stay forever' clinics.
GeneSight is an adjunct after failed trials, not a first-visit upsell.

Florida telehealth psychiatry is available to established and new patients located in Florida at the time of the visit, subject to licensure and clinical appropriateness. An in-person evaluation at the Hollywood office may be recommended when a physical exam, vital signs, or a more complete diagnostic interview is needed.
